We are all so accustomed to our parents caring for us that it is so very hard when the roles reverse. We see our parents as young, strong, and invincible. But, as they age we notice how weak they have become and how much they rely on their children to help them now. Life has started changing and no longer do the parents care for the children, but the children are beginning to care for the parents. One of the things that goes along with caring for aging parents is how to handle the mobility problems that ensue when a parent begins using a walker, cane, wheelchair, or is bedridden for months at a time. Fortunately, there are wheelchair lifts, vertical lifts, dumbwaiters, and even residential elevators that assist with these problems and allow individuals to help their parents through the aging process and allow them their mobility even if it is in a different fashion than before.

It might seem difficult to make the decision to purchase a wheelchair lift or even a residential elevator for your aging parents; however these decisions have positive benefits for your parents. For example, if you have a wheelchair lift then your parents can go up and down stairs themselves without additional help. Dealing with your aging parents is not easy and while it is hard to see your parents grow old and need assistance you dont have to despair too much because there are so many options available. Your parents can stay mobile through the many devices available and you wont have to worry about them being stuck in a single room with no way to get around.

I was a CNA in a local nursing home, as well as a housekeeper, and have seen first hand the types of abuse and neglect that can occur when a home is understaffed and over worked. Do not put up with inferior nursing care… Our elderly deserve caring, quality care. After all, this is the home they will reside in until the day they depart this earth.

Someresidents are aware of their poor care and the pain, discomfort and inconvenience caused by it. Others are not. But whether the individual is aware or not, their loved ones usually are, and the pain and emotional suffering borne by loved ones may be as great as that suffered by the injured resident.

Visiting often, paying close attention to your loved ones comments or reluctance to certain staff is important, as well as paying close attention to the staff is vital to your loved ones piece of mind and comfort. residents who often complain just may have a valid reason for that complaint, but often times go unnoticed for one reason or another.

Medical Assistance (MA), or Medicaid, is a federal/state insurance program that pays for health and treatment services for children and adults with disabilities. Some of these services include such things as health screenings, therapies, behavioral support services, transportation, and home healthcare. Medical Assistance was authorized on July 1, 1969, under the Social Security Act, Title XIX, Grants to States for Medical Assistance Programs. It is funded partly by the states and partly by the federal government. The code of federal regulations, Title 42, The Public Health and Welfare, is another important source of law that states the federal regulations pertaining to medical assistance. Medical Assistance is a free program and anyone can apply for Medical Assistance, however, not everyone with disabilities is eligible for Medical Assistance benefits. Each state establishes its own eligibility standards, benefits package, provider requirements, payment rates, and program administration under broad federal guidelines. This paper will provide an overview of Medical Assistance eligibility requirements, services, and the rights of children that are Medical Assistance eligible, ages 3-21.

In order to meet the federal MA qualifications, the child with disabilities must meet the U.S. citizen/alien requirements for MA, fit into one of the categories that MA establishes, and meet income and resource requirements (Pennsylvania Health Law Project, 2003). In order to be MA eligible in Pennsylvania, one must be a resident of the state or a covered immigrant (Pennsylvania Health Law Project, 2003). There are several categories within MA that cover persons with disabilities and they often have different requirements (Pennsylvania Health Law Project, 2003). Children with disabilities that are eligible for Social Security Income (SSI) are automatically eligible for MA. Children with disabilities that are categorized as severely disabled and are not on SSI are also eligible for MA, regardless of their parents income and assets and if their own personal income is less than the poverty level (in 2003 was $749/month). The Department of Welfare calls this category the loophole category (Pennsylvania Health Law Project, 2003). In this category, if a childs income exceeds the poverty level, the child is still possibly eligible for CHIP (Childrens Health Insurance Program) or may be eligible for a Waiver program if the child requires a higher level of care (Pennsylvania Health Law Project, 2003). Most children in Pennsylvania with severe physical, mental, or behavioral disabilities qualify for MA services (Education Law Center, n.d.).

Although a child with disabilities may be receiving special education services and have a particular diagnosis, it does not mean that the child meets the disability standards for Medical Assistance (Parents Involved Network of Pennsylvania, n.d.). The eligibility standards for special education are different from the MA disability standards. However, a childs Evaluation Report (ER) can be an excellent source that documents factors that are considered in the Medical Assistance disability standard (Parents Involved Network of Pennsylvania, n.d.).

Children with disabilities that qualify for MA receive what is called an ACCESS card, which they can utilize for the purchase of a variety of prevention and treatment services. Anything that is medically necessary must be provided and children that are eligible are entitled to these services. Except through a Waiver program, MA does not cover any educational or rehabilitation services. MA services are free and there are no co-pays or additional charges. If a family has private medical insurance, this must be used first for the child with disabilities (Education Law Center, n.d).

Examples of behavioral health services covered by MA include residential treatment facilities, behavioral services (wrap-around services, therapeutic support staff), medication for behavioral problems, and partial hospitalization programs (Education Law Center, n.d.). Examples of health services that are covered by MA include nursing care at home or at school, therapies, communication devices, medical equipment, and personal care services for children who need assistance with self-care skills, such as eating or dressing (Education Law Center, n.d.). MA must also provide eligible children with any services that are necessary to meet the childs physical or behavioral needs during the day, such as a nurse, a specialized hearing aid, or a therapeutic support staff person. Many of these services, however, are also considered related services based on IDEA regulations, and school districts are responsible for listing these services on the childs IEP even if the services are being covered by MA (Education Law Center, n.d.).

MA recipients have certain rights in Pennsylvania. The following list of rights is an excerpt from the Pennsylvania Law Projects website:

1. The right to receive and file an application on the same day that you ask for it.
\r2. The right to bring someone with you to help you with the MA application.
\r3. The right to have an application completed by a friend, relative, or official of a hospital, agency, etc. if you are ill or physically or mentally unable to do so.
\r4. The right to translation services and translated written material.
\r5. The right to receive coverage beginning with the 3rd month before the application, if you qualify for retroactive MA.
\r6. The right to have DPW quickly issue a MA card if you have an immediate need for medical services.
\r7. The right to receive medically necessary treatment and services without discrimination based on national origin, race, color, sex, or disability.
\r8. The right to free choice of MA enrolled health care providers unless you are enrolled in the Health Choices Program.
\r9. The right to be treated with dignity and respect.
\r10. If you are homeless, the right to apply for MA even if you have no address.
\r11. The right to have the MA programs explained to you and to receive help in determining the best possible coverage for which you qualify.
\r12. The right to prior notice of, and a fair hearing to contest, any decision by the MA agency or an MA HMO to deny, terminate, or reduce benefits.

All decisions regarding the rejection of MA or the reduction or termination of MA must be provided to recipients in writing and with instructions on how to appeal. Recipients have 30 days to file appeals, but if the appeal is filed within 10 days of the date of termination or reduction notice, MA benefits must continue until there is an outcome to the appeal (Parents Involved Network of Pennsylvania, n.d).

I think its pretty safe to say that we all want the best for our loved ones, and we generally tend to assume that no one will care for that loved one as well as we can. But dementia is more than just a bit of short-term memory loss. Dementia can and will impact the very personality of that aging parent or grandparent. Indeed, the person you have known your entire life may not be the person you are inviting into your home. And special consideration should be given as to whether or not your home is equipped to provide for them.

Physical Requirements:

Are there are a lot of stairs in your home? Dementia can affect balance and equilibrium, not to mention strength. If you live in a 2-story home, will your loved one be able to safely navigate the stairs? Or will they require assistance? If they require assistance, and their bedroom is on the second floor, you have just placed yourself in the position of having to provide 24 hour, round the clock, supervision and assistance.

Meal Preparation:

Your loved one has been preparing their own meals for the past 50, 60, or 70 years. Why shouldnt they be able to do so now? The truth is, while they are more than likely to recall how to cook a meal, it doesnt mean they can do so without supervision. The problem lies in the severity of the short-term memory loss. It is quite common for sufferers of dementia to forget they have something on the stove if they are even slightly distracted. A phone call, or coming across an old photograph can completely distract them from the task at hand.

Medication:

As our loved ones age, numerous medical conditions can arise that require medication. And that medication regimen can be quite complex. Some pills should be taken with food, some on an empty stomach. One prescription can have 2 or 3 doses per day, while others are taken only at bedtime. The monitoring of medication is critical for dementia patients. What can complicate this further is an inability to recall if they have taken a medication. And as the disease progresses, you may find that their ability to discern what medication should be taken disappears completely.

Consider; they have just taken a pain medication. But five minutes later, they still hurt. They have no memory of having already taken the pills, and so they go in search of more. Even more frightening can be an inability to distinguish one medication from another. The bottles all appear similar, for many the printing is just too small to make out, and so they take whatever is closest at hand. From personal experience I know this is the case. Just as you did when your children were small, all medications must be locked away. But this is not a child. They can reach where your children obviously could not. And most childproof closures will be easily opened. Securing medications is absolutely critical.

Lifestyle:

Is your lifestyle truly compatible with the loved one youre bringing home? Bear in mind that for dementia sufferers change is very difficult. As their memory slips away, the new and unfamiliar can become increasingly disconcerting. For instance, has that parent or grandparent spent the last 10 or 20 years living alone? If so, consider carefully before bringing them into an environment resounding with the enthusiasm and ruckus of bustling children.

The perpetual stillness and quiet of their previous environment will make them ill prepared for the constant sound and motion of their new environment. Simple things like the closing of a door, or someone walking up the stairs will be enough to wake them from a sound sleep, and that seems to be a particularly vulnerable time because as they wake, they will likely fail to recognize their environment. Imagine waking alone, in the dark, in a place you do not recognize, to strange and unfamiliar sounds. That can be a terrifying experience for anyone.

Mood Swings:

A common symptom of dementia is severe mood swings. Without warning this pleasant, loving individual can lash out in anger or succumb to uncontrolled weeping. In my experience, this was often associated with TIAs. A TIA is a \”warning stroke\” or \”mini-stroke\” that produces stroke-like symptoms but no lasting damage. However TIAs can produce wide fluctuations in blood pressure and bring on several days of unpredictable mood swings. While adults can reason through to understand these mood swings, can your children? Are you prepared to subject your child to completely illogical complaints and expectations that have no rhyme or reason? And is your child of an age that he or she can comprehend the nature of these events and harbor no resentment or hurt feelings. It is an admirable goal to seek to build a loving relationship between generations, but it is quite another to expect a child to respond with love to irrational anger and unreasonable expectations.

Progression of the Disease:

Finally, regardless of your present circumstances, you must be prepared for the progression of the disease. The level at which they function today may and probably will change over time. It can be heart wrenching to make the determination that you can no longer care for that parent or grandparent. And explaining to someone you have loved your entire life that they are no longer welcome to live with you can dredge up a host of painful emotions like guilt, anger and resentment.

It is here where you need to speak honestly and openly with your health care provider. They can provide the stepping-stone to long-term care by interceding on your half behalf with both the dementia sufferer and other family members. By pointing out the need for more intensive medical attention than anyone could hope to provide in a home environment, they can relieve you of that burden of guilt youve been shouldering for so long and help you to find a facility that can provide the care that you simply cannot.

Care Advocate - It is important to determine who you would like to be your care advocate now. This will designate the person who will look after the way your care operates if long term care is necessary.

End of Life Planning- No one wants to do this, but it is sometimes necessary to ensure that your arrangements, where you would like to die, and other wishes are followed according to your desires.

Legal Arrangements and Documents - Legal documents are necessary to ensure that everything is followed according to your wishes. This includes power of attorney and estate planning.

Financial Information - You will want to provide a list of insurance plans, income, and assets that could assist in paying for your long term care planning.

Create a Resource Book - This book should include any companies, doctors, and facilities to be given to family and car advocates.

Checklist - This allows those involved to make the right choices for your care.

Tell Your Family - It is ultimately important to communicate your long term care planning with your family and loved ones. This saves everything from being a surprise, if and when the time comes.
